Prototype Plan Documents

First Actuarial Corporation (FAC) has its own standardized and nonstandardized prototype documents. These documents can be used by executing an adoption agreement. Prototype documents are available for Profit Sharing, Money Purchase, Defined Benefit Pension, Target Benefit and 401(k). FAC can provide individually designed plans as well.

By the end of the plan year that begins in 2001, all qualified plans must be restated to comply with a number of recent law changes that are collectively known as GUST. FAC has updated all of its prototype documents to comply with GUST.
